Membership Plan

Your membership begins with an initial 12-month term and automatically continues month to month afterward. You may request cancellation by providing 30 days’ notice to Golden Smiles Dental Studio. Cancellation and refunds will follow applicable law, including the return of any unearned prepaid membership fees.

Monthly membership payments are due on the scheduled billing date. If a payment is unsuccessful, the member will have a 7-day grace period to update their payment method and bring the account current. Membership benefits and discounts may be temporarily paused until all outstanding payments are paid.

A $15 late payment fee may be applied to accounts that remain unpaid after the 7-day grace period. Membership benefits will resume once the account is brought current.

Your member savings apply immediately to eligible treatment plans totaling $2,500 or less. For treatment plans totaling more than $2,500, your full initial 12 months of membership fees must be paid before the discount is applied. The $2,500 threshold is based on the total treatment plan price before discounts. Membership must be active when treatment is provided. Savings cannot be combined with promotions, model pricing, or other discounts.

Membership benefits and discounts cannot be combined with private dental insurance benefits for the same service. Membership fees and services included in your membership will not be billed to insurance. For separately charged treatment, you may choose member pricing or insurance billing, subject to your insurance plan and our billing policies. If you choose insurance billing, the membership discount will not apply to that treatment. Golden Smiles is out of network, and insurance coverage or reimbursement is not guaranteed.

This agreement is not health or dental insurance.. It provides the listed services and savings directly through Golden Smiles Dental Studio. If you need treatment from a specialist or another outside provider, our membership benefits and discounts do not apply to that provider’s services. Their fees and insurance arrangements are separate.

Luxe cleanings cover routine dental prophylaxis only. If you need a deeper cleaning or periodontal therapy, those services are billed separately. We will explain the recommended treatment and any additional fees before proceeding, with eligible membership discounts applied.

Membership benefits do not roll over into the next membership year. Unused benefits cannot be exchanged for cash, credit, or other services. This does not affect refunds of unearned prepaid membership fees under the cancellation policy.

Third-party financing options are available for eligible treatment, subject to lender approval and terms. Any interest or financing fees are separate from your membership fees.

If you move during your initial 12-month membership term, please contact us. Relocation requests will be reviewed individually, with any applicable adjustment or refund calculated on a prorated basis. Your cancellation rights and right to a refund of unearned prepaid fees still apply.

Membership does not waive missed-appointment or no-show fees. These fees apply to members and nonmembers according to the practice’s written appointment policy. Any applicable fee and notice requirements will be disclosed before your appointment.

Membership pricing and benefits may be updated for each new 12-month membership year. We will provide at least 30 days’ written notice before changes take effect. Changes will not apply during your current membership year, and you may cancel before the new terms begin.
